Diana Lachapelle

Diana Lachapelle, CPA

Chief Executive Officer

Diana joined the HealthSouth Concord in 2012 as controller with more than 20 years of progressive financial and operational management experience. Her interest in healthcare began as a public accountant with Ernst & Young where she served a number of acute care hospitals and healthcare facilities throughout New Hampshire. In addition, she served as the director of finance for Timberland and controller for Nashua Corporation focusing on financial planning, cost containment, internal controls and operational improvements. She received her bachelor’s degree in business administration from the University of New Hampshire and is a Certified Public Accountant in New Hampshire. Outside of work, Diana is an active member of the National Charity League’s Southern New Hampshire Chapter.

Dr. Tollman

James Tollman, MD, FHM

Medical Director

James Daniel Tollman, M.D., FHM is the founder, president, and CEO of Essex Inpatient Physicians (EIP). In September 2012 he became the medical director of HealthSouth Rehabilitation Hospital in Concord New Hampshire, where he currently oversees all aspects of clinical management.

Prior to his work as hospitalist, Dr. Tollman was a primary care physician with The Lahey Clinic in Burlington, Massachusetts. He attended Tufts University Family Practice Residency program and received his medical degree from the University Of Nebraska College Of Medicine. He completed undergraduate studies in biology at Carleton College in Minnesota. He served as an active duty infantry officer in the U.S. Army for five years.

Dr. Tollman is a diplomat in the American Board of Family Medicine. His professional society memberships include the Society of Hospital Medicine- Fellow in Hospital Medicine, Inaugural Class of May 2009, the Phi Rho Sigma Medical Society- Iota Chapter President from 1997 through 1998, The American Medical Society, The American Academy of Family Medicine and The Massachusetts Academy of Family Physicians.

He holds unrestricted medical licenses in the states of New Hampshire, Massachusetts and Connecticut.

James Tollman, M.D. is an independent private practice physician whose clinic is located at the hospital.

Wendi Guillette

Wendi Guillette, OTR

Director of Therapy Operations

Wendi has practiced in the field of occupational therapy for 17 years. After obtaining a bachelor’s degree in occupational therapy from the University of New Hampshire, she completed her internship at HealthSouth Rehabilitation Hospital. This experience solidified her desire to focus her career on adult rehabilitation. Since then, she has worked in numerous healthcare settings including acute care, home care, sub-acute and outpatient. Wendi rejoined the HealthSouth team as the inpatient therapy team leader in 2002 and was promoted to director of therapy operations in 2011. In addition to her work at HealthSouth, she also serves on the New Hampshire Occupational Therapy Governing Board and is a member of the New Hampshire Brain Injury Provider Council.

Christopher Kenny

Christopher D. Kenny, D.O.

Director of Rehabilitation

Christopher D. Kenny, D.O. is dual-board certified in physical medicine and rehabilitation as well as electrodiagnostic medicine. He is a physiatrist at New Era Medicine and is the director of rehabilitation at HealthSouth Rehabilitation Hospital in Concord. Dr. Kenny received his Bachelor of Science in molecular biology from Grove City College. He received his Doctor of Osteopathic Medicine from the College of Osteopathic Medicine of the Pacific in Pomona, California on the Army Health Profession Scholarship Program and completed his transitional-year internship and residency in physical medicine and rehabilitation at Walter Reed National Military Medical Center. Prior to joining the medical team at HealthSouth Concord, Dr. Kenny was stationed in Ft. Hood, Texas at Carl R. Darnall Medical Center where he served as chief of the Interdisciplinary Pain Management Center, chief at the Physical Medicine Clinic, officer in charge of the Chiropractic Clinic and PROFIS medical officer in the 566th Medical Brigade Defense CBRNE Response Force at Ft. Hood, Texas. In addition, he earned the Expert Field Medical Badge. He holds an active, unrestricted medical license in the state of New Hampshire.

Myra Nixon

Myra Nixon

Director of Human Resources

Myra brings more than 10 years of human resource experience in a healthcare setting to HealthSouth Rehabilitation Hospital. In the course of a diverse 18-year progressive career in hospitality, she has gained experience to be a proactive management executive, with strong leadership skills in strategic planning and implementing policies and programs based on company oriented goals. Myra serves on the board of directors for the Manchester Community Health Center and is a member of the New Hampshire Hospital Association.

Kate Palmieri

Kate Palmieri, BSW

Director of Case Management

Kate has been employed by HealthSouth Rehabilitation Hospital for more than 12 years. She joined HealthSouth Rehabilitation Hospital in 2000 and was promoted to the director of case management in 2004. Kate holds a bachelor’s degree in social work from Keuka College in Keuka Park, New York. She completed her internship in at Loretto Geriatric Center in Syracuse, New York. Kate is a member of the Case Management Society of New England and participates in the Case Management and Social Workers Leadership meetings of New Hampshire.

Brenda Paradis

Brenda Paradis, RN-BA

Chief Nursing Officer

Brenda joined HealthSouth Rehabilitation Hospital in 2008 as the director of quality and risk management. She brings more than 25 years of diverse healthcare management experience including acute care and various leadership roles at WellPoint. Brenda earned her Six Sigma certification in 2005 and is known for her focus on customer service, operations and process improvement.

Dr. Salmanullah

Muhammad Salmanullah, MD

Associate Medical Director

Dr. Salmanullah received his MBBS from Dow Medical College in Karachi, Pakistan and completed his internship and residency at Berkshire Medical Center in Pittsfield, Massachusetts. He is certified by the Educational Commission for Foreign Medical Graduates.

Prior to joining the medical team at HealthSouth, he was a founding member of the Hospitalist Program at Missouri Delta Medical Center and an internist with SEMO Health Network in Sikeston, Missouri. Dr. Salmanullah has had experience as a research fellow at the Children’s Hospital Boston and Harvard Medical School, Division of Nephrology in Boston Massachusetts and as a research associate at the University Of Maryland School Of Medicine, Division of Nephrology, in Baltimore, Maryland. He has been published in many medical journals for his research in renal transplantation and he was awarded a research grant from the National Kidney Foundation.

Dr. Muhammad Salmanullah has joined the medical team at HealthSouth Rehabilitation Hospital, in Concord New Hampshire as the associate medical director.

Susan Tanner

Susan Tanner

Business Development Director

Susan comes to HealthSouth Rehabilitation Hospital with an extensive background in healthcare marketing and administration. She completed her undergraduate degree in business administration at the University of Massachusetts. Her range of experience is broad and includes the design, planning and operations of healthcare facilities. Known especially for her collaborative style, Susan builds successful partnerships and relationships with healthcare providers and community groups across the continuum. The hallmark of her work is maintaining a balance on customer service and effective operations. She most recently served as president of the board of directors of one of the largest not-for-profit hospice organizations in Connecticut.
