Safer Transitions through Patient and Family Engagement

During healthcare transitions, patients and their families are the constant. Engaging them in the plan of care reduces safety risks for the individual.
Using this simple concept can be an effective strategy to improve the transition of care for the patient along the healthcare continuum. This event is designed to be a collaborative program featuring area providers including Concord Hospital, Concord Regional Visiting Nurse Association and HealthSouth Rehabilitation Hospital of Concord. The discussion will include a case study review of the journey through the continuum for a stroke survivor and their family.
- Demonstrate how engaging with patients and families can be used as a strategy to improve safety during transitions.
- Identify areas where patients, families and organizations can improve communication during transitions.
- Recognize opportunities and determine steps for improved communication and partnership with patients, families and the healthcare community.
